2025 BCCPAC AGM Proxy Form Submission

Use this form to assign your PAC's/DPAC's proxy (ie. voting and/or speaking rights) at the 2025 BCCPAC AGM.

If there are any errors or questions arising from the information submitted on this form, BCCPAC will contact the Submitter, Executive members and Delegate.

After the submission has been validated, BCCPAC will contact the Delegate, Executive members and Submitter to confirm the status of the proxy.

IMPORTANT: PROXIES ARE NON-TRANSFERABLE. This means you cannot share your Lumi login credentials with another individual; however, a proxy can be revoked and a new proxy form can be submitted. For more information, read the FAQ above or contact our office.
